Most of the time, msm means men who have sex with men. It is a label based on what someone does sexually, not what they call themselves, so researchers and health folks use it. One roast though, if you see it online and you are not in a health context, people are probably just repurposing it the way everyone repurposes acronyms, like it is a free costume.
